Autonomous Nurse Practitioner

Job Summary
We are seeking a highly skilled and motivated Autonomous Nurse Practitioner to join our team performing house calls. Your expertise will support a wide range of medical services, from primary care to transitional care, managing acute issues and chronic diseases, ensuring patients receive timely, effective, and compassionate care. This position offers flexible work and hours seeing patients in the home setting. This is a 1099 contract position.

Responsibilities

• Conduct thorough patient assessments, including vital signs, physical examinations, and medical history reviews to determine appropriate care plans.
• Manage complex cases independently, mostly geriatrics, ensuring adherence to best practices and clinical guidelines.
• Utilize electronic health record (EHR) systems such Tebra/Kareo or eClinicalWorks for accurate documentation of patient encounters and medical records.
• Coordinate with multidisciplinary teams to optimize patient outcomes and also help prevent hospitalizations.
• Option to provide telehealth if needed to assess urgent needs and determine appropriate levels of care remotely.

Experience

• Proven experience as a Nurse Practitioner.
• Demonstrated proficiency in working with geriatrics, assisted living residents, hospice & palliative medicine patients.
• Familiarity with EMR/EHR systems; telehealth experience is a plus.
• Knowledge of CPT coding, ICD-10 coding standards for billing purposes is preferred.
• Experience with care in the home setting or in assistive living facilities is also preferred.

Pay: $70.00 per hour
