Position Summary
The Behavioral Health Case Manager plays a vital leadership role in supporting residents, guiding direct care staff, and ensuring the successful implementation of individualized treatment goals within our Psychiatric Supportive Living Program.
This position works under the direction of the Operations Manager and serves as the bridge between leadership, residents, families, and direct support staff. The Case Manager ensures that treatment plans, behavioral strategies, house expectations, and daily programming are carried out consistently by both residents and staff.
This is a highly collaborative role that requires strong leadership, organization, accountability, excellent communication skills, and the ability to coach others while maintaining compassionate, resident-centered care.
Essential Responsibilities Resident Support
- Build professional, therapeutic relationships with residents.
- Assist residents in developing independent living skills.
- Support residents in achieving individualized treatment goals.
- Conduct regular one-on-one meetings with assigned residents.
- Monitor emotional, behavioral, and psychiatric stability.
- Observe and document changes in behavior or functioning.
- Help residents develop healthy routines and structure.
- Encourage participation in therapy, appointments, workshops, employment, education, exercise, and community activities.
- Teach problem-solving, communication, emotional regulation, budgeting, hygiene, and social skills.
- Assist residents in navigating benefits, community resources, and outside services.
Staff Leadership & Coaching
- Work directly under the guidance of the Operations Manager.
- Ensure all staff understand and consistently implement individualized resident plans.
- Provide coaching and guidance to Direct Support Professionals regarding resident interventions and expectations.
- Reinforce consistency among staff to promote predictable, therapeutic care.
- Follow up on assignments delegated by the Operations Manager and ensure completion.
- Model appropriate communication and de-escalation techniques.
- Assist with onboarding and mentoring new staff.
- Promote teamwork, professionalism, and accountability.
Clinical Coordination
- Maintain communication with therapists, psychiatrists, families, treatment teams, and community providers.
- Participate in multidisciplinary treatment planning.
- Assist in creating and updating individualized goals and behavioral support strategies.
- Track resident progress toward established goals.
- Monitor medication compliance and communicate concerns to the appropriate team members.
- Assist with discharge planning and transitions of care.
- Help coordinate appointments, transportation, and follow-up care.
Documentation
- Complete accurate and timely progress notes.
- Maintain organized resident records.
- Document incidents, behavioral observations, interventions, and follow-up actions.
- Ensure documentation meets organizational standards and timelines.
- Report significant concerns promptly to leadership.
Program Operations
- Help maintain a structured, therapeutic living environment.
- Monitor resident participation in daily programming.
- Assist in organizing recreational, educational, and life skills activities.
- Support house meetings and goal-setting sessions.
- Help identify barriers to resident success and develop practical solutions.
- Collaborate with leadership to continuously improve program quality.
Qualifications
- Bachelor's degree in Psychology, Social Work, Human Services, Behavioral Health, or a related field preferred.
- Experience 1-2 years with Mental Health Clients
- Relevant experience may substitute for education.
- Experience working with adults experiencing serious mental illness preferred.
- Knowledge of schizophrenia, bipolar disorder, depression, anxiety disorders, substance use, and trauma-informed care.
- Strong leadership, organizational, and communication skills.
- Ability to coach and support staff while maintaining positive working relationships.
- Ability to remain calm during behavioral crises.
- Strong documentation and computer skills.
- Valid California Driver's License with an acceptable driving record.
- CPR and First Aid Certification (or willingness to obtain).

Pay: $30.00 - $33.00 per hour
