Behavioral Health Case Manager Social Worker
Position: Behavioral Health Case Manager
Status: Full Time, Non-Exempt
Northlake Behavioral Healthcare System is seeking a Behavioral Health Case Manager to support inpatient psychiatric patients through care coordination, discharge planning, and connection to community resources. This role works closely with an interdisciplinary treatment team to improve patient outcomes, reduce length of stay, and ensure safe transitions to the next level of care.
- Coordinate care from admission through discharge
- Develop and manage individualized discharge plans
- Arrange follow up care including outpatient, IOP, PHP, residential, or community services
- Participate in treatment team meetings and track patient progress
- Communicate with patients, families, and external providers
- Facilitate psychoeducational and skills based groups under supervision
- Support legal and community coordination as needed
- Complete accurate and timely documentation in the electronic medical record
- Assist with crisis response and patient safety support
- Bachelor’s degree in Social Work required
- 1 year behavioral health or healthcare experience preferred (inpatient or acute care preferred)
- BLS and CPI required (training provided if needed)
- Strong communication and organization skills
- Ability to manage a fast paced caseload
- Knowledge of behavioral health systems and discharge planning
- EMR proficiency and attention to documentation
- Strong commitment to team care model and ability to collaborate effectively in an interdisciplinary setting
- Empathy, compassion, and a patient-centered approach to care
This is a fast paced, collaborative role focused on helping patients transition safely and successfully back into the community.
