POSITION SUMMARY: Under the supervision of integrated BH Manager provide the integrated behavioral health services/case management at AHS Family Health Center (AHSFHC).
DUTIES AND RESPONSIBILITIES:
- Conduct comprehensive client assessments to collect functional, environmental, psychosocial, financial, employment, housing, educational and health information as appropriate regarding patient’s need.
- Assess and provided case management services to clients as per the needs based on the intake.
- Assist BH team in scheduling patient appointments (with the psychiatrist) and process patient requests in EMR under the guidance of the psychiatrist/BH team.
- Facilitate client access to community resources including substance abuse and MH services, recreational activities, housing, food, clothing, school programs, vocational opportunities and other relevant supportive services.
- Assist clients in contacting social supportive networks.
- Responsible for the adherence and maintenance of Mental Health policies and procedures for the behavioral health program.
- Responsible for a caseload, providing case management services.
- Participates in the development of the AHSFHC ITP healthcare plan.
- Attends administrative team meetings, care coordination meetings and organization-wide staff meetings on a monthly basis.
- Maintains professional competence by pursuing opportunities for continuous learning, attending continuing education offerings, reading professional literature, and engaging in other activities, which develop new knowledge and skills.
- Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, Mental Health Confidentiality Act and other State and Federal laws).
- Performs other duties as assigned.
QUALIFICATIONS:
· Bachelor’s degree in relevant human services field plus at least one year experience in providing case management.
· Ability to effectively present information both orally and written to administration.
· Ability to work as a team member.
· Computer literacy is required and ability to process patient records electronically.
· Ability to work with a culturally diverse team.
· Must be willing to work flexible hours as required by position.
· Urdu/Hindi or other Asian language - preferred.
Pay: $42,000.00 - $50,000.00 per year
