HopeCentral is a faith-based 501(c)(3) non-profit clinic and serves all families in Southeast Seattle regardless of income, status, or belief system. We launched in 2014 as a primary care practice with the mission of “sharing the love of Jesus through excellent, enhanced primary care for the entire community.” Since 2015, we have been an Autism Center of Excellence for Washington state and have provided pediatric primary care with a focus on special healthcare needs.
Summary
At HopeCentral, we believe it is important to care for the whole person; mind, body, and spirit. Toward that end, we have adopted a multidisciplinary team-based approach to health and wellness. Our behavioral health clinician is a core member of the collaborative care medicine (CoCM) team, which works together with physicians and a consulting psychiatrist to address children’s mental health care needs and to facilitate improved functioning.

Licensure:
- Masters-level Licensed Independent Clinical Social Worker (LICSW) in Washington State
- Masters-level Licensed Mental Health Counselor (LMHC or LMFT)
- Masters-level Licensed Mental Health Counselor Associate (LMHCA)
- Masters-level Licensed Social Work Associate Independent Clinical (LSWAIC)
Qualifications:
- 2 years experience working with children and families in a clinical setting.
- Excellent communication skills, both verbal and written.
- Ability to work collaboratively within a multidisciplinary team environment.
- Experience in collaborative care model preferred
- Knowledge of evidence based treatment strategies for children
- Working knowledge of neurodevelopmental disabilities, including autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), intellectual disability, and specific learning disorders.
- Able to provide both in person and telehealth visits
Responsibilities
- Support the behavioral and physical health care of patients on an assigned patient caseload. Closely coordinate care with the patient’s medical provider and, when appropriate, other mental health providers.
- Screen and assess patients for common mental health disorders. Facilitates patient engagement and follow-up care.
- Provide parent education and coaching about common behavioral health problems, home behavioral management strategies, and available treatment options.
- Systematically track treatment response and monitor patients (in person or by telehealth) for changes in clinical symptoms and treatment side effects or complications.
- Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment.
- Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, RUBI, motivational interviewing, and other treatments as appropriate.
- Facilitate treatment plan changes for patients who are not improving as expected in consultation with the medical provider and the psychiatric consultant and who may need more intensive or more specialized mental health care.
- Develop and complete maintenance self-management plan with patients who have achieved their treatment goals and are soon to be discharged from the registry.
- Participate in case management, coordination with schools and other community support providers to enhance outcomes, taking into account social determinants of health
Job Type: Full-time
Pay: $75,000.00 - $85,000.00 per year
