Behavioral Health Clinician & Health Coach

We are building a next-generation "payvider" — a physician-led concierge primary care practice and health plan in one — with an obsessive focus on the member experience. Because we both deliver care and own the cost of it, every incentive is aligned around keeping members healthy: radically better, coordinated care that members love, delivered efficiently by cutting the friction, waste, and unnecessary utilization that define most of healthcare today. Backed by experienced healthcare operators and investors, the organization is building the clinical and operational foundation for a direct-to-consumer concierge health plan launching in the DC metro area.

POSITION SUMMARY

We are seeking a Licensed Clinical Social Worker (LCSW) to serve as our founding Behavioral Health Clinician & Health Coach, delivering integrated behavioral health care within a high-touch, technology-enabled primary care practice. As the first behavioral health hire in the organization, this clinician will provide licensed behavioral health services embedded directly in the care POD while also delivering proactive health coaching that helps members build lasting, preventive habits.

This is a dual-scope role anchored on licensed clinical work. The clinician will operate as a behavioral health professional first — providing screening, brief evidence-based interventions, and short-term therapy in close collaboration with the physician-led care team — and will flex into a health coaching capacity focused on behavior change, lifestyle, and chronic disease support. The role is ideal for a behavioral health clinician who values whole-person, relationship-driven care, integration with primary care, and the opportunity to help build a modern behavioral health and coaching program from the ground up.

CORE RESPONSIBILITIES

  • Provide integrated, embedded behavioral health care within the care POD, collaborating closely with physicians and APPs through warm handoffs and shared care plans
  • Conduct behavioral health screening (e.g., PHQ-9, GAD-7) and assessment, and deliver evidence-based interventions and therapy
  • Establish clear escalation and referral pathways for members who need a higher level of behavioral health care
  • Deliver proactive health coaching focused on behavior change, lifestyle, and chronic disease management, with an emphasis on prevention and reducing downstream utilization
  • Provide high-touch, concierge-level support through a combination of virtual visits, in-person sessions, and asynchronous messaging
  • Participate in proactive outreach to members with behavioral health needs, care gaps, and elevated risk
  • Partner with care navigators and patient allies, who own logistics and system navigation, while this role owns clinical behavioral health and behavior-change coaching
  • Champion an obsessive focus on the member experience — hospitality, responsiveness, and proactive communication at every touchpoint
  • Help shape behavioral health and coaching pathways, standards, and program design as the organization scales
  • Utilize digital tools, structured intake pathways, and AI-supported workflows to improve efficiency and the member experience

CLINICAL MODEL

The practice is designed around smaller member panels and high-access care delivery, with efficiency coming from technology and operations so that clinician time goes to members. Each POD will support approximately 500–750 members and include a physician, APP(s), a medical assistant, and an embedded behavioral health clinician who also provides health coaching. The care model emphasizes:

  • Integrated behavioral health delivered alongside primary care
  • Preventive and relationship-driven care

  • Async-first workflows for low-acuity needs
  • Proactive care coordination and navigation
  • Reduced administrative burden through technology-enabled operations

PREFERRED QUALIFICATIONS

  • Active Licensed Clinical Social Worker (LCSW) credential
  • Active or eligible licensure in Washington, DC, Maryland, and/or Virginia, with willingness and ability to become licensed in additional states to support remote members
  • Comfort with multi-state licensure and telehealth-based behavioral health delivery
  • Experience delivering integrated or collaborative behavioral health within a primary care or value-based care setting preferred
  • Training in brief, evidence-based interventions (e.g., CBT, motivational interviewing) and short-term therapeutic models
  • Experience or interest in health coaching, behavior change, and chronic disease support
  • Strong communication skills and comfort with digital-first member engagement
  • Interest in innovation, care redesign, and early-stage healthcare ventures
  • Collaborative, team-oriented approach to whole-person care
  • Commitment to delivering evidence-based, high-touch behavioral health care
