Job Summary
The Behavioral Health Consultant (BHC) is a highly flexible, core member of the integrated primary care team who utilizes an adaptive and patient-centered approach to address the psychosocial needs of primary care patients in a community health center. The BHC provides timely access to behavioral health services for a broad range of mental health and physical health needs to address the health of the whole person. Working within the primary care behavioral health model (PCBH), same-day behavioral health visits paired with medical primary care visits (warm handoffs) allow us to reach patients who might otherwise not receive mental health care and reduce disparities in access to behavioral health. BHCs also follow patients for ongoing treatment, allowing for a balanced clinical schedule. BHCs receive training and supervision in the PCBH model and evidence-based intervention from their supervisor to promote high quality care and professional development.
As our health center continues to grow and to prioritize our presence in community spaces to reach underserved individuals with many barriers to accessing care, this new BHC position will be split across our integrated primary care clinic in Wyoming and our new primary care clinic housed within Mel Trotter Ministries. Our clinic at Mel Trotter Ministries offers primary care, dental hygiene services, psychiatry, and case management support, in addition to behavioral health, to patients experiencing homelessness. This role offers the clinician an opportunity to gain experience with highly integrated behavioral health care, to work with patients who have substance use disorders, and to implement tailored, evidence-based interventions to improve equity in access to health care for an underserved population.
