$16.00/$17.00 to start

Overview:

We are seeking dedicated, organized, and experienced Behavioral Health Integration (BHI) Specialists to join our team. As a BHI Specialist, you will be responsible for overseeing and coordinating the delivery of behavioral health integration services to our patients. This is a crucial role in our organization, as you will play a key role in ensuring the well-being and mental health of our patients.

Duties:

● Obtain consent and enroll patients in the BHI Services program.

● Establish & maintain a continuous relationship with the patient and a collaborative, integrated relationship with the rest of the care team.

● Takes part in regular review of clinical status of patients getting BHI services.

● Tracking patient follow-up and progress using the registry, with proper documentation; participation in weekly caseload consultation with the psychiatric provider.

● Ongoing collaboration with and coordination of the patient’s mental health care with the treating physician or other qualified health care professional and any other treating mental health providers.

● Other review of progress and recommendations for changes in treatment, as indicated, including medications, based on recommendations supplied by the psychiatric provider.

● Monitoring of patient outcomes using validated rating scales.

● Relapse prevention planning with patients as they achieve remission of symptoms, other treatment goals and prepare for discharge from active treatment.

Experience:

- Minimum of [1] years of experience in behavioral health or skilled nursing facility or assisted living facility experience.

- Excellent communication, computer, and interpersonal skills.

- Ability to work effectively as part of a multidisciplinary team.

- Experienced with computer software and other electronic devices.

- Must have experience with administrative functions such as charting and data entry.

- Strong ability to work well independently.

- Reliability and accountability are essential for this role.

- Current licensure or certification as a health professional (Medical Assistant, CNA or nursing)

Job Type: Full-time

Pay: $16.00 - $18.00 per hour
