Position Overview

Premier Home Healthcare is seeking a skilled and compassionate Behavioral Health Practitioner to support youth with high behavioral and mental health needs in both home and community settings.

This role focuses on crisis stabilization, behavioral support, and safety monitoring while delivering structured, person-centered care in alignment with Minnesota DHS 245D standards.

Schedule

  • Full-time and part-time positions available
  • 8-hour shifts available
  • 3-day, 12-hour shifts available

Compensation

  • $24.00 to $26.00 per hour (depending on experience)

Key Responsibilities and Essential Functions (DHS 245D Aligned)

  • Provide direct 1:1 supervision and support per the Coordinated Service and Support Plan (CSSP)
  • Ensure health, safety, and supervision at all times
  • Implement person-centered and trauma-informed care
  • Identify early signs of escalation and provide timely intervention
  • Use de-escalation and positive support strategies
  • Complete accurate and timely documentation, including progress notes and incident reports
  • Follow all 245D policies, including abuse prevention and reporting requirements
  • Support participation in community-based activities to promote inclusion and independence
  • Maintain professional boundaries and ethical conduct
  • Communicate effectively with families, supervisors, and interdisciplinary team members

Required Qualifications

  • 1 to 2+ years experience working with:
  • High behavioral youth
  • Mental health conditions
  • Crisis intervention
  • High school diploma or equivalent
  • Ability to pass a MN Department of Human Services background check
  • Valid driver’s license and reliable transportation

Preferred Qualifications

  • Degree in Psychology, Social Work, Human Services, or related field
  • Training in:
  • Crisis Prevention Intervention (CPI)
  • De-escalation techniques
  • Trauma-informed care
  • Experience in:
  • In-home or residential behavioral services
  • Mobile crisis services and or Community Health Worker experience

  • Cultural awareness and sensitivity
  • Sound judgment and professionalism

Physical Requirements

  • Ability to lift and assist up to 50 pounds
  • Ability to stand, walk, and remain active for extended periods (8 to 12-hour shifts)
  • Ability to respond quickly in crisis situations
  • Ability to bend, kneel, reach, and assist with daily activities
  • Sufficient physical stamina and mobility to safely support youth

Work Environment

  • Client homes and community-based settings
  • Exposure to behavioral escalation, including verbal aggression
  • High-stress situations may occur
  • Must maintain professional composure and safety awareness at all times
