The Behavioral Health Provider (BHP) delivers integrated, team-based behavioral health services within a primary care setting using the Primary Care Behavioral Health (PCBH) model, while also providing psychotherapy as part of the broader continuum of care. This role emphasizes flexible, patient-centered service delivery, including same-day access, brief interventions, and longitudinal therapy, based on patient need and program priorities, with a primary focus on brief, population-based care within the PCBH model.
The BHP partners closely with primary care providers and the interdisciplinary team to address mental health conditions, substance use concerns, and behavioral factors that impact chronic disease management and overall well-being. Services are grounded in evidence-based practices and may range from brief, solution-focused interventions within primary care visits to more traditional psychotherapy, supporting both population health goals and individualized care.

- Utilize validated screening tools (e.g., PHQ-9, GAD-7, AUDIT-C) and apply measurement-based care to inform treatment decisions and track outcomes
