Youth Case Manager

Schedule: Monday through Friday

Location: Rimrock Arizona 85635

Hourly pay rate: $22.00 - $24.00 hourly

We’re looking for a compassionate and organized Case Manager to support youth in a residential treatment setting. In this role, you’ll help guide each client’s care, working closely with families, clinicians, and community partners to support positive outcomes.

What You’ll Do

Serve as the main point of contact for assigned clients
Coordinate communication between treatment team members and families
Help develop and update individualized treatment plans
Track progress and ensure goals are being met
Maintain accurate, timely documentation
Support and guide families throughout treatment
Assist with discharge planning and next steps for care

Strong organization and communication skills
Ability to manage multiple clients at once
Knowledge of behavioral health and trauma-informed care
A team-oriented mindset and passion for helping others

Requirements

High school diploma or equivalent required; 1 year of relevant experience preferred
Post-secondary education in a related field is a plus
Ability to maintain confidentiality and follow HIPAA guidelines
Strong communication and interpersonal skills
Ability to work in a fast-paced environment and adapt to change
Basic computer skills and familiarity with electronic medical records
