Bereavement Coordinator

Job Overview

The Bereavement Coordinator is responsible for planning, implementing, and coordinating bereavement services for families and loved ones of hospice patients. This role provides emotional support, grief education and follow-up care while ensuring compliance with hospice regulations and standards.

Responsibilities

• Develop and manage the hospice bereavement program.
• Provide grief support and counseling to family members and caregivers before and after a patient's death.
• Conduct bereavement risk assessments and establish individualized plans of care.
• Facilitate grief support groups, memorial services and educational programs.
• Maintain regular contact with bereaved families for up to 13 months following a patient's death.
• Coordinate referrals to community resources and mental health professionals as needed.
• Maintain accurate documentation and ensure compliance with hospice regulatory requirements.
• Collaborate with the interdisciplinary hospice team to address family and caregiver needs.

Qualifications

• Master's degree in Social Work, Counseling, Psychology, Human Services, or a related field required.
• Proven experience in hospice care, bereavement counseling, social services, mental health or related healthcare settings preferred.
• Familiarity with medical terminology, ICD coding (ICD-9/10), CPT coding, and medical documentation standards.
• Strong understanding of grief, loss and end-of-life care.
• Excellent communication, organizational, and interpersonal skills.
• Ability to provide compassionate support to individuals and families experiencing loss.
• Valid driver's license and reliable transportation.

Pay: $29.88 - $35.98 per hour
