Bilingual Case Manager

The Case Manager completes intake assessments, develops a case plan, and provides or connects participants to identified resources and services. Programming and services may include, but are not limited to, effective emotional or trauma related therapies, housing services, life skills, employment training, job placement services, family engagement, and other support services. Our Case Manager will carry a caseload of participants at and will earn one hour of PTO for every 40 hours. The case manager will also effectively communicate the organizations mission to Spanish speakers.

DUTIES AND RESPONSIBILITIES

  • Provide crisis intervention services to individuals or families experiencing a crisis situation
  • Build trusting relationships in order to create buy in for social services
  • Provide a single point of contact for the participants who self-select social services
  • Conduct individualized intake assessments and plans
  • Be available to answer the phone d talk with group members and provide after-hours crisis services, including phone-based consultations and preliminary intake services
  • Follow up regularly with group members to see if there are areas in which further help, or guidance is needed
  • Track the progress of each group member in terms of programmatic completion
  • Work 1-on-1 with participants to identify their needs, develop plans to meet those needs, and monitor their progress over time. Build and maintain healthy relationships with participants
  • Utilize assessment results to inform the development of the participants’ Individual Development Plan/Case Plan
  • Provide guidance and monitor individual participant activities as outlined in the case plan to track progress of individual participants toward attainment of identified goals
  • Coordinate links to appropriate support services and provide appropriate referrals such as transportation assistance, basic needs and emergency assistance, behavioral health support, mental health support and access to other support services
  • Support social emotional learning; anti-bullying prevention; suicide prevention, and other evidence-based programs.
  • Assist in identifying comprehensive, data-driven prevention strategies that work most effectively to prevent and reduce firearm violence.

QUALIFICATIONS

  • Minimum of 2 years relevant experience in case management
  • Demonstrated working knowledge of supportive services and resources
  • Ability to build and maintain relationships and partnerships
  • Demonstrated experience working with sensitive populations and victims of firearm violence and administering assessments
  • Knowledge of case management principles, objectives, standards, and methods, including motivational interviewing
  • Familiarity with support service agencies in Cook County
  • Ability to work with and engage individuals of diverse backgrounds
  • Excellent verbal and written communication skills in Spanish and English
  • Demonstrated ability to advocate, organize, and problem-solve
  • Demonstrated ability to maintain confidentiality with sensitive information
  • Ability to take a collaborative, team-oriented approach.
  • Ability to be accessible both during traditional and non-traditional work hours, including nights and weekends

Pay: $4,000.00 per month
