ONE LIFE UNLIMITED is currently seeking to hire a bi-lingual Care Manager. The goal of this position is to help individuals with developmental disabilities to maintain the best quality of life. Care Managers work closely with Families and Care Providers to facilitate the provision of services, including, Respite, Habilitation and Attendant Care. This position will conduct member intakes, coordinate services, act as liaison to Families, Care Providers and the DDD (Division of Developmental Disabilities) and continuously monitor provided services to ensure that the individual's unique needs are met.

Skills/Requirements

This position does require travel using your own vehicle. Must have reliable transportation.

Bilingual in English/Spanish is a requirement for this position. Must be FLUENT in both languages.

SKILL in: Excellent oral and written communication; interviewing employees; counseling employees and interacting with clients and family members; coordinating and facilitating meetings; Microsoft Office; Microsoft Windows.

ABILITY to: Work independently; organize and manage caseloads; document case files and information within required/mandated timelines; maintain confidentiality; pass a background check, obtain a level 1 fingerprint clearance card, flexible working hours including weekends, if required

Experience/Education

One of the following are required

• A Bachelor's degree in Social Science, Health Education, Education, (i.e., psychology, social work, health sciences, early childhood education) or other closely related field from an accredited college or university
• Two years in a professional work environment OR a combination of education and experience which must include one year of Case Management at the professional level.

Job Type: Full-time

Pay: $43,000.00 - $55,000.00 per year
