Position Summary:
We are seeking a highly motivated and qualified Clinical Care Coordinator to join our Kerrville team. This role plays a key part in our Chronic Care Management (CCM) and population health programs, working directly with patients and providers to ensure quality care, documentation compliance, and positive health outcomes. The position blends clinical coordination with healthcare administration and public health practice.
Key Responsibilities:
Clinical & Chronic Care Management:
Coordinate and deliver services under the Chronic Care Management (CCM) program.
Develop and maintain individualized care plans in accordance with Medicare guidelines.
Provide patient education and chronic disease self-management support.
Monitor and track chronic conditions, including outreach and follow-up to improve outcomes.
Maintain accurate and compliant Medicare documentation.
Administrative & Population Health Functions:
Oversee quality reporting, EHR workflow optimization, and care gap closure.
Track and analyze population health data to support value-based care goals.
Collaborate with providers and staff on Medicare compliance and regulatory reporting.
Participate in QI/QA activities and help align clinic performance with state and federal benchmarks.
Master’s degree in Public Health (MPH), Health Administration, Nursing, or a related health sciences field.
Background in medicine (e.g., MBBS) or nursing is highly preferred.
International medical graduates (IMGs) with a doctoral-level degree (such as MBBS, MD, or equivalent) from outside the United States are strongly encouraged to apply.
Experience in Chronic Care Management, Medicare documentation, or public health coordination.
Proficiency with EHR systems and clinical workflows.
Knowledge of healthcare terminology, HIPAA, and CMS guidelines.
Bilingual (English/Spanish).
Preferred Skills:
Prior experience in value-based care models.
Strong communication, patient engagement, and interdisciplinary collaboration skills.
Work Schedule & Compensation:
Full-time, Monday–Friday.
