Bilingual Nurse Navigator

Purpose:
The Bilingual Nurse Navigator (RN/LPN) works directly with a provider or providers to guide FMC patients by providing clinical support, care coordination, patient education, and follow-up services. This position serves as an extension of FMC’s medical providers to ensure treatment success.

Responsible: This position supports proactive, patient-centered care by improving communication, enhancing continuity of care, assisting with treatment management, coordinating follow-up services, and helping patients successfully navigate care.

Accountable: The Bilingual Nurse Navigator is accountable to the FMC Head Nurse.

Job Scope and Status: Non-exempt employee (Hourly) working four (4) to five (5) days per week (36–40 hours per week), with employee benefits, paid time off, and overtime, additional pay, or flex time where approved and appropriate. Pay Range: LPN: $22–$28 per hour | RN: $30–$35 per hour

Job-Specifics: The role serves as a key connection between FMC providers and patients by supporting care coordination, patient education, clinical follow-up, and healthcare navigation. This position provides bilingual communication support for Spanish-speaking patients, assists patients with understanding diagnoses, medications, treatment plans, and follow-up instructions, and helps identify and address barriers impacting healthcare access. Responsibilities include conducting patient outreach, coordinating referrals and appointments, supporting transitions of care, assisting with medication reconciliation, performing nursing assessments, providing chronic disease education, supporting preventive health initiatives, and documenting all patient interactions within the electronic health record. This position also participates in quality improvement activities by monitoring patient engagement, referral completion, healthcare access, and outcomes.

Collaboration, Communication, and Reporting: Collaborate effectively with providers, nursing staff, behavioral health professionals, community health workers, volunteers, and external partners to support coordinated and successful patient care. Provide timely communication regarding patient needs, clinical concerns, follow-up activities, and barriers to care. Attend staff meetings, training sessions, and organizational initiatives while supporting FMC’s commitment to teamwork, continuous improvement, and exceptional patient service. Represent FMC professionally when interacting with patients, families, healthcare partners, and community organizations.

Qualifications:

• Graduate of an accredited nursing program with a current Tennessee Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in good standing
• Current CPR/BLS certification required
• Bilingual English/Spanish fluency required
• Minimum of two (2) years of clinical nursing experience preferred, with experience in primary care, family medicine, chronic disease management, care coordination, patient navigation, or ambulatory healthcare preferred
• Experience with electronic health records, telehealth technologies, and healthcare technology platforms preferred
• Strong clinical assessment, communication, patient education, organizational, and critical-thinking skills required
• Ability to work independently while effectively collaborating within an interdisciplinary care team

Pay: $35.02 - $37.03 per hour
