This full time position is responsible for navigating clients through the various systems and into the appropriate level of substance use disorder treatment. This position will manage access into SUD services within a network of providers for both SSP residential and withdrawal management. This position requires proficient computer skills in order to document and bill in the Electronic Health Records. This position requires working with a team of managers and counselors to implement, maintain and evaluate relevant, applicable and evidence-based approaches, and provide consistent support for participants’ successful entry into treatment services.
The Bilingual Pre-admit Specialist Specific Duties and Responsibilities
Collaborating with referring parties daily to collect data on eligible ASAM Brief Screening recipients as well as coordinating intake dates
Transcribing, monitoring and reporting all ASAM Brief Screening data to maintain contract requirements.
Connecting with referring parties to coordinate any prescriptions so that client medications are sent to the appropriate pharmacy utilized by each program.
Checking with referring partners about care or follow-up needed for any acute or chronic medical conditions, mobility issues, current withdrawal status or any other medical discharge orders related to continuity of care as client moves from facility to treatment program.
Coordinating program intake time dosage times for those clients receiving methadone to ensure clients are able to dose prior to leaving jail facility and can coordinate with Janus methadone for continued dosage for clients entering treatment.
Communicating and coordinating with mental health team to provide complete wrap-around services and any additional support necessary for client success.
Completing ASAM Brief Screenings over the phone, via telehealth or in person.
Connecting those who are eligible to Santa Cruz Medi-Cal in order to fund treatment.
Refer clients to the appropriate level of care based on ASAM brief results.
Document assessment, care coordination and referrals in the electronic health record.
Responsible for answering the phone, checking benefits and scheduling assessments for both SSP residential and the withdrawal management.
Coordinate with various community partners to avoid duplication of services and explore appropriate treatment placement.
Bilingual preferred (English/Spanish)
Experience working with co-existing conditions (SUD and Mental Health)
Experience performing assessments, writing clinical summaries and treatment plans
Demonstrated skills and understanding of effective treatment of addiction and recovery
In depth understanding of the ASAM assessment and corresponding Level of Care
Commitment to the goal of empowerment of low-income, incarcerated persons from diverse backgrounds
Background and understanding of trauma informed care preferred
Background and understanding of Motivational Interviewing preferred
Computer literate in the following software preferred (training available): Microsoft Word, Outlook, Avatar, ARMS, Excel
Demonstrated ability to organize and prioritize activities
Demonstrated problem-solving, conflict resolution and crisis intervention skills
Demonstrated ability to use good judgment and decision-making
Ability to work both independently and collaboratively
Organizational ability to keep accurate records
