Blended Case Manager

Job Overview
As a Blended Case Manager, you will play a pivotal role in supporting individuals and families through comprehensive case management services. You will engage with clients to assess their needs, develop personalized plans, and provide ongoing support in navigating various systems including behavioral health, child welfare, and workforce development. Your role will empower clients to achieve their goals and improve their overall well-being.

What you’ll do

• Conduct thorough assessments to identify client needs and strengths, developing tailored service plans.
• Provide on-call service to clients experiencing challenges.
• Collaborate with community resources.
• Mentor clients in personal development and life skills while promoting self-sufficiency.
• Maintain accurate documentation of client progress, service delivery, and outcomes.

Total Compensation Package

• $59,000 which includes: health benefits, 401k Match, life insurance, paid holidays, PTO, bonus, and salary of $40,000.

Requirements

• Bachelor’s degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education.
• Excellent communication skills with the ability to build rapport with diverse populations.
• Ability to work collaboratively within multidisciplinary teams while maintaining client confidentiality.
• Act 33/34 clearances, FBI clearance, Valid Pa driver's license, Reliable transportation, Proof of Pa insurance and Physical health examination
