Position Overview

We are seeking a Board-Certified or Board-Eligible Palliative Care Physician to join our growing Portland team. This position offers the best of both worlds — autonomy and flexibility, paired with the support of a highly collaborative, multidisciplinary care team.

You’ll conduct daily rounds at your own pace, supported by a dedicated Nurse Practitioner (NP), providing holistic and patient-centered care across LTAC and acute care facilities.

Key Responsibilities

  • Deliver expert clinical care and leadership in LTAC and acute care settings
  • Manage complex symptoms and collaborate with your NP on individualized treatment strategies
  • Develop, implement, and oversee proactive care plans tailored to each patient’s needs
  • Conduct daily rounds with NP support for documentation and care coordination
  • Partner with specialists and primary care providers to ensure continuity and alignment of care goals
  • Facilitate thoughtful discussions on care preferences, treatment options, and advance care planning
  • Oversee smooth transitions of care, including hospital discharges and follow-ups
  • Maintain accurate, timely documentation in the electronic health record (EHR) system

Qualifications

  • M.D. or D.O. from an accredited medical school
  • Board Certified or Board Eligible in Palliative Care or Hospice & Palliative Medicine
  • Active Oregon medical license (or eligibility for licensure)
  • Current DEA registration
  • Prior experience in LTAC or acute care preferred
  • Excellent communication, collaboration, and interpersonal skills
