Position Overview

We are seeking a Board-Certified or Board-Eligible Palliative Care Physician to join our expanding team in Covina, CA. This position offers autonomy, flexibility, and consistent support from an experienced Nurse Practitioner (NP). You’ll play a key role in providing comprehensive palliative care to patients in LTAC and acute care facilities, collaborating with an interdisciplinary team to design and implement effective, compassionate care plans.

Key Responsibilities

  • Deliver high-quality, patient-centered palliative care in LTAC and acute care environments
  • Support patients and families through complex medical decisions and serious illness discussions
  • Partner with an NP to manage symptoms, prescribe treatments, and monitor progress
  • Conduct daily rounds with NP assistance for documentation and follow-up
  • Collaborate with specialists, hospitalists, and primary care providers to ensure continuity of care
  • Facilitate thoughtful discussions regarding care goals, treatment options, and advance directives
  • Maintain accurate and timely documentation within the electronic health record (EHR)
  • Promote smooth transitions of care, including discharge planning and follow-up coordination

Qualifications

  • M.D. or D.O. from an accredited medical school
  • Board Certified or Board Eligible in Palliative Care or Hospice & Palliative Medicine
  • Active California medical license and DEA registration
  • Prior experience in LTAC or acute care preferred
  • Strong communication and interpersonal skills
  • Compassionate, empathetic, and patient-centered approach to care
