Join our dynamic healthcare team as a Brooklyn Occupational/Physical Therapist specializing in homecare! Duties
- Conduct comprehensive assessments of patients’ physical and occupational needs within their home environment, including geriatrics, orthopedics, and post-acute care cases.
- Develop, implement, and adjust individualized care plans that align with patient goals and medical requirements.
- Deliver hands-on therapy sessions utilizing techniques such as trigger point therapy, mobility training, and functional exercises to improve strength, flexibility, and overall mobility.
- Collaborate with physicians, nurses, caregivers, and other healthcare professionals to coordinate discharge planning and ensure continuity of care across inpatient, outpatient, hospital, and community settings.
- Document patient progress accurately using Electronic Medical Record (EMR) systems while maintaining compliance with healthcare regulations.
- Educate patients and their families on injury prevention, physiotherapy exercises, and home modifications to promote safety and independence.
- Manage acute care situations when necessary by applying physiology knowledge and medical terminology to adapt treatment strategies effectively.
Qualifications
- Valid licensure as an Occupational Therapist or Physical Therapist with relevant state certifications.
- Proven experience in home & community care environments with a strong understanding of outpatient clinics, hospital inpatient units, or home health services.
- Demonstrated expertise in discharge planning, care plan development, and acute care procedures.
- Familiarity with EMR systems for documentation and patient management.
- Knowledge of anatomy, physiology, orthopedics, geriatrics, pediatrics, and trigger point therapy techniques.
- Strong communication skills to effectively educate patients on treatment plans and safety measures.

Pay: $70.00 - $100.00 per hour
