Position Overview
A physician-led medical practice is seeking experienced California-licensed Nurse Practitioners specializing in Behavioral Health and/or Musculoskeletal (MSK) care to participate in an advanced, asynchronous Telehealth care model.
This opportunity is designed for independent contractor clinicians engaged by a physician-led medical practice, operating within a structured, compliance-driven environment that supports autonomous clinical decision-making.
Clinical Model
This role operates within a non-traditional Telehealth framework focused on:
- Asynchronous patient engagement (no traditional live visit requirement)
- Structured intake utilizing branching clinical logic
- Integrated behavioral health and MSK care pathways
- Continuous monitoring through validated outcome measures (e.g., PHQ-9, GAD-7, pain scales)
- Defined escalation protocols for higher acuity or synchronous intervention
Scope of Responsibilities
- Review and assess patient-submitted clinical histories and symptom data
- Determine patient appropriateness for program participation
- Develop and manage evidence-based treatment plans
- Prescribe therapies in accordance with California regulations (if applicable)
- Monitor patient progress using standardized outcome tools
- Escalate high-risk or complex cases per established protocols
- Maintain accurate, compliant, and audit-ready documentation
- Exercise independent clinical judgment within a structured care model
Required Qualifications
- Active, unrestricted California Nurse Practitioner license
- Board Certification (FNP, PMHNP, or equivalent)
- Active CMS-855I enrollment (approved; not pending)
- Active NPI (Type 1)
- DEA license (if prescribing applicable therapies)
- Demonstrated clinical experience in:
- Behavioral Health (required)
- MSK care (preferred or willingness to integrate)
- Ability to practice independently within a protocol-driven environment
Preferred Experience
- Prior Telehealth or asynchronous care delivery experience
- Management of chronic pain, anxiety, depression, or functional MSK conditions
- Familiarity with validated outcome measures (PHQ-9, GAD-7, NRS, etc.)
- Experience in integrated care models (behavioral + physical health)

- Fully remote (must be licensed in California)
- Flexible, contractor-driven schedule
- Asynchronous workflow with defined turnaround expectations
- Engagement aligned to program operations (not volume quotas)
Practice & Compliance Structure
- Independent contractor clinicians are engaged by a physician-led medical practice
- Providers maintain full clinical autonomy and decision-making authority
- Care delivery must align with:
- California Telehealth regulations
- Medicare enrollment and billing compliance requirements
- Established clinical protocols and escalation policies

- Currently practicing in behavioral health and/or MSK within California
- Fully credentialed and enrollment-ready (including active CMS-855I)
- Comfortable with asynchronous, non-visit-based care delivery
- Strong documentation, compliance, and clinical judgment skills

Engagement Type
- Independent Contractor (1099)
