Social Worker Trainee $35,124.00Social Worker I $38,724.00Social Worker II $47,064The CAP/DA Case Manager is responsible for administering and coordinating services under the Community Alternatives Program for Disabled Adults (CAP/DA). This program enables eligible adults (age 18 and older) who qualify for nursing facility level of care to remain safely in their private residences. The position focuses on person-centered case management, service coordination, and ongoing monitoring to ensure participants receive cost-effective, high-quality care that supports independence and well-being.

Program Eligibility & IntakeReceive, review, and process referrals and service requests for individuals seeking participation in the CAP/DA Waiver programConduct comprehensive assessments to determine eligibility and level of care needsCollaborate with medical providers to obtain required documentation supporting eligibilityCase Management & Care CoordinationDevelop, implement, and maintain individualized Plans of Care (POC) in accordance with NC Medicaid guidelinesEnsure all waiver and non-waiver services are accurately documented at initial enrollment, annually, and during revisionsCoordinate services with providers, caregivers, and community resourcesSupport consumer-directed care options and educate participants on service delivery modelsMonitoring & Quality AssuranceConduct regular and routine monitoring of beneficiaries to ensure health, safety, and well-beingPerform annual reassessments to determine continued eligibility and service needsIdentify and address risks through appropriate interventions and risk mitigation strategiesComplete required quality assurance reporting and maintain accurate case recordsCommunity Outreach & EducationEducate the elderly and disabled adult community, families, and caregivers about the CAP/DA Waiver programPromote awareness of available services and supportsAssist individuals and families in navigating long-term care optionsProgram Development & ComplianceDevelop and maintain case management procedures in compliance with NC Medicaid standards and local policiesShare procedures and updates with providers and community partnersMaintain an Advisory Committee to support program development, resource expansion, and service improvementEnsure compliance with all federal, state, and local regulations governing CAP/DA services
