Job Description
Florida State Nurse Registry, LLC is currently seeking a compassionate, experienced, and dependable Skilled Cardiac Nurse (RN) to provide high-quality, patient-centered nursing care to individuals with cardiovascular conditions in a home health setting throughout Osceola and Orange County.

This role focuses on assessment, monitoring, education, and intervention for patients with cardiac diagnoses while ensuring compliance with physician orders, agency policies, and state/federal regulations. The ideal candidate will have experience working with cardiac patients in home health, private duty, rehabilitation, or acute clinical settings.

Key Responsibilities

Clinical Care

• Perform comprehensive nursing assessments focused on cardiovascular health, including CHF, CAD, post-MI, and arrhythmias
• Monitor vital signs, cardiac symptoms, and disease progression
• Administer medications and treatments as prescribed, including cardiac-specific therapies
• Assess for signs of complications such as fluid overload, chest pain, or dyspnea and intervene appropriately
• Provide wound care when applicable, including post-surgical cardiac patients

Care Coordination

• Collaborate with physicians, cardiologists, and interdisciplinary team members to develop and update care plans
• Communicate changes in patient condition promptly to appropriate providers
• Ensure accurate and timely documentation in accordance with regulatory standards

Patient & Family Education
Educate patients and caregivers on:

• Disease process and symptom management
• Medication adherence
• Diet recommendations including low sodium and fluid restrictions
• Lifestyle modifications and cardiac rehabilitation principles
• Reinforce discharge instructions and support hospital readmission prevention

Requirements

• Active, unencumbered Florida Registered Nurse (RN) License
• Minimum 1–2 years of nursing experience, preferably in cardiac care, home health, or acute care
• Current BLS (Basic Life Support) Certification
• Strong knowledge of cardiac conditions and treatments
• Reliable transportation
• Strong clinical assessment and critical thinking skills
• Excellent communication and patient education abilities
• Ability to work independently and manage time efficiently
• High level of accountability and attention to detail
• Compassionate, patient-focused approach aligned with care standards

As required by AHCA, all applicants must comply with the Florida Care Provider Background Screening Clearinghouse requirements.

Preferred Qualifications

• Experience in home health or skilled nursing visits
• Familiarity with OASIS documentation
• ACLS Certification preferred
• Experience with telehealth or remote patient monitoring
