Classification: Part Time/1099 Contractor

Work Structure: Fully Remote

Schedule/Shift: Mon-Fri; 4 hours/day - either morning or afternoon (EST)

Team: Clinical Operations - Intensive Cardiac Rehabilitation

Reporting to: Clinical Staff Manager
Location: United States

Compensation: $20 per 30-minute session

Position Overview

The Cardiac Rehab Onboarding EP is a patient-facing role responsible for onboarding, education, and early engagement of patients enrolled in the Intensive Cardiac Rehabilitation program after the initial session. This role supports patients during the first sessions by providing goal setting, program education, progress monitoring, and technical assistance to ensure patients can successfully participate in both individual and group sessions.

The Onboarding EP works collaboratively with a multidisciplinary care team, including nurses, exercise physiologists, and patient experience specialists to support patients across various stages of recovery following a cardiac event.

Key Responsibilities

Patient Onboarding & Education

  • Conduct first session and early follow-up sessions to orient patients to the cardiac rehabilitation program including completing fitness testing
  • Educate patients on program structure, expectations, and participation requirements, including group and individualized sessions
  • Reinforce the importance of regular attendance, engagement, and completion of the program
  • Support patients in understanding next steps in their care journey

Technical Support & Session Readiness

  • Assist patients with basic device and platform troubleshooting
  • Guide patients step-by-step on how to join virtual sessions (audio, video, platform navigation)
  • Ensure patients are technically prepared to independently access future sessions
  • Identify and escalate unresolved technical issues to the appropriate team

Goal Setting & Behavioral Coaching

  • Support patients in establishing realistic, personalized health and recovery goals
  • Provide motivation and emotional support, addressing common barriers such as anxiety, low confidence, or disengagement
  • Utilize basic behavioral and coaching techniques to promote adherence and sustained participation

Group Engagement Support

  • Enroll patients into appropriate group education sessions
  • Educate patients on the value and expectations of group participation
  • Identify and address barriers to group session attendance (technical, scheduling, understanding)

Collaboration & Documentation

  • Communicate patient progress, challenges, and concerns to the care team as needed
  • Maintain accurate, timely documentation of sessions, goals, and patient interactions
  • Record relevant data for program tracking and quality improvement efforts

Skills & Qualifications

Education/Certifications

  • Bachelor's or Master's degree in Health, Exercise Science, Kinesiology, or a related field
  • CPR and First Aid certification (required)
  • ACSM Certification- Clinical Exercise Physiologist/ Exercise Physiologist or AACVPR-CCRP (required)

Experience

  • Experience working in a clinical, rehabilitation, or patient support setting preferred
  • Background in health coaching, fitness, or exercise physiology strongly preferred
  • Experience supporting cardiac or chronic care populations is a plus
  • Experience with motivational interviewing, patient engagement and customer service.

Core Skills & Attributes

  • Strong communication and patient education skills with an ability to build rapport with patients in a short time
  • Comfort providing basic technical support in a virtual care environment
  • Knowledge of cardiovascular health, behavior change principles, and lifestyle modification
  • Empathy, patience, and ability to support patients with varying physical and emotional needs
  • Strong organizational skills and ability to work independently in a remote setting

Role Summary

This role is designed to reduce friction at program entry, improve early patient engagement, and support smooth transitions into ongoing cardiac rehabilitation care by combining patient education, coaching, and technical readiness. Success in this role is defined by high patient satisfaction rate & meeting the required performance metrics.
