Cardiologist - part-time

We are seeking a part-time (20-30 hours per week), remote-based, board-certified Cardiologist to support our value-based, in-home care delivery model through collaborative management of patients with cardiovascular disease. This role is responsible for providing clinical oversight and support to providers delivering in-person and virtual patient care, including evaluation and treatment plan optimization for cardiovascular conditions. Other key responsibilities include escalation support for our interdisciplinary care teams when they identify a rising risk patient and collaboration & communication with network PCPs and specialists. The ideal candidate will bring clinical expertise, a strong understanding of value-based care principles, and a passion for improving patient outcomes at the individual patient and overall population level, along with exceptional communication and collaboration skills and strong alignment with our mission.

Key Responsibilities:

  • Collaborate with ConcertoCare interdisciplinary care teams across states to deliver exceptional care for patients with cardiovascular disease and complex health needs.
  • Conduct clinical case reviews of patients with cardiovascular disease in collaboration with nurse case manager and provider, including clinical assessments, diagnostic test results, outside medical records, and current treatment plan, followed by development of recommendations for treatment plan optimization and support for implementation of recommendations.
  • Provide consultative guidance per collaborative care model and ongoing clinical oversight and escalation support to interdisciplinary care teams in care team huddles, written consultation format, and with ad hoc outreach and communication.
  • Collaborate with ConcertoCare providers, Primary Care Providers, community-based specialists, and interdisciplinary teams to optimize patient care plans and support implementation of care plans.
  • Support ConcertoCare patients & providers both indirectly and directly with specialist-to-specialist communication needs for network providers as indicated.
  • Identify opportunities to reduce unnecessary hospitalizations and promote preventative, home-based interventions for patients with cardiovascular disease.
  • Contribute to the development of clinical pathways and protocols tailored to in-home care settings.
  • Support virtual patient care delivery on ad hoc basis for highest-risk patients, especially for patients who lack access to care from network cardiologist.

Required Qualifications:

  • MD or DO with active board certification in Cardiovascular Disease.
  • Active, unrestricted medical license in any state and willingness to apply for multi-state licensure.
  • Minimum 3 years of clinical cardiology experience including care of elderly patients and patients with complex care needs.
  • Prior experience working with nurse practitioners, physician assistants, or other interdisciplinary care team members in delivering team-based care to patients.
  • Strong communication and documentation skills.
  • Proficiency with EMRs, data dashboards, and virtual care platforms.
  • Ability to work independently in a remote environment.

Preferred Qualifications:

  • Prior experience with care management, population health, and/or value-based care delivery preferred.
  • Advanced training or board certification in Geriatric Medicine or in Hospice and Palliative Care preferred.
  • Experience leading clinical initiatives and driving clinical performance outcomes preferred.

Base Salary/ Wage Range $155,000 to $235,000. Compensation for the role is commensurate with the candidate’s qualifications, skills, competencies, and experience and may fall outside of the range shown.
