We partner with community cardiologists to deliver longitudinal, virtual specialty care for patients with heart failure and cardiometabolic disease, using a multidisciplinary care team and an AI-enabled, device-agnostic platform. The Role

We are hiring a Nurse Practitioner with heart failure expertise to play a central clinical role in our virtual care program.

As an ISHI NP, you will practice at the top of your license, managing a longitudinal panel of patients with heart failure, hypertension, diabetes, CKD, and related cardiometabolic conditions. You’ll lead medication optimization, respond to device- and data-driven signals, and collaborate closely with cardiologists, pharmacists, nurses, and navigators to deliver proactive, high-touch care.

What You’ll Do

• Provide longitudinal virtual specialty care to patients with heart failure and cardiometabolic comorbidities
• Conduct telehealth visits focused on symptom assessment, risk stratification, medication initiation and titration (with emphasis on GDMT), and patient education
• Review and act on device, EHR, and lab data (e.g., weight, BP, PAD/CardioMEMS trends, heart rate, labs) to guide timely interventions
• Lead clinical escalations and coordinate with cardiologists and the multidisciplinary care team
• Support post-hospital discharge transitions to reduce readmission risk
• Collaborate with referring cardiologists, PCPs, pharmacists, nurses, and navigators
• Document care clearly and efficiently using ISHI’s digital tools
• Participate in care huddles, quality reviews, and ongoing program refinement

What We’re Looking For

Required - Active Nurse Practitioner license in Arizona- 2+ years of clinical experience in cardiology, heart failure, or a related specialty outpatient setting- Comfort managing patients with heart failure and cardiometabolic disease- Strong clinical judgment and ability to work independently in a virtual environment- Excellent communication skills with patients, caregivers, and physician partners

Nice to Have - Dedicated heart failure experience- Experience with virtual care or remote patient monitoring- Spanish fluency- Interest in clinical program design, quality improvement, or digital health innovation

Schedule & Compensation

• Remote (U.S.-based)
• Full-time, Monday–Friday, 8:00am–5:00pm PT
• Annual base salary range: $130,000 – $155,000

This salary range represents ISHI Health’s good-faith estimate of the base compensation for this role. Actual compensation will be determined based on experience, licensure, scope of responsibility, internal equity, and applicable law.
