Care Coordinator

Birch Meadows is a new Senior Care facility in Three Rivers, Michigan. We consider it an honor to serve our residents and we never take that privilege lightly. Our family has been in the senior care business since 1960 and we recognize the challenges families face when needing additional care for loved ones. Birch Meadows is looking for competent Care Coordinator to assist our staff and care for our residents and in a professional and compassionate manner. It’s a very satisfying and rewarding job, since our elderly residents and their families are thankful for the high-quality assistance and care they receive. The ideal candidate will be patient and compassionate, with excellent communication skills and reliable attendance. You should be able to lead by example and perform a variety of tasks to help the team, as well as the residents. Our goal is to be intentional while caring for the whole person.

Responsibilities include, but are not limited to:

• Med Management
• Caregiving alongside our team, this would include floor shifts
• Oversight of Resident Care
• Communication with Doctors, Families, Nurses, Pharmacy, Staff etc....
• On Call Rotation
• Mandation Rotation
• Care plan updating and oversight
• Weekend Rotation

Responsibilities vary based on shift worked, but employees will be trained for all shift duties.

Qualifications:

• Compassion and respect for the elderly
• Prior care experience
• Prior management or leadership experience
• Strong multi-tasking abilities, as well as being aware of things happening around you
• Physical endurance and strength
• Interpersonal skills, including exceptional patience and listening skills
• Professionalism
• Ability to work in partnership with other employees to maintain excellent employee relations, high employee morale, and world class senior care
• Commitment to working a full shift, arriving on time, and working overtime as needed

Pay: $16.00 - $19.00 per hour
