Care Coordinator CNA/MA/CMA - Antlers, OK

We are seeking a dedicated and organized CNA, MA or CMA to fill a Care Coordinator position. The selected candidate would oversee the daily operations of On-Site Wounds within our contracted facilities throughout Oklahoma, statewide. This position coordinates clinical activity, manages the flow of patient care and acts as a liaison between the facility and our providers in order to enhance the quality of patient care and ensure healthcare services are delivered in an efficient manner. The ideal candidate must have superior organizational skills and a strong commitment to patient-centered care. Local travel required.

Responsibilities

• 
  
  Provide necessary support to rounding providers through coordination of their schedule, pre-rounding preparation, effective use of communication tools and clinical documentation.
• 
  Collaborate with healthcare providers to facilitate effective communication between them, the patients, the facility and other stakeholders.
• 
  Monitor patient flow and coordinate with facility in such a manner as to minimize wait times and enhance service deliver.
• 
  Maintain accurate patient records and ensure compliance with health regulations.
• 
  Oversee inventory management for any needed medical supplies and/or equipment.
• 
  Plan, organize and execute educational in-services for facility staff regarding work-flow, best practices and other healthcare related topics.

  Promote wellness among residents/patients by assisting with patient education initiatives.

Qualifications

• 
  
  Applicant must have had prior experience working in a medical setting.
• 
  Previous experience in nursing home and assisted living facilities preferred.
• 
  Strong interpersonal skills with the ability to work effectively with diverse populations.
• 
  Ability to manage multiple tasks simultaneously while maintaining a high level of professionalism.
• 
  Excellent organizational skills and attention to detail.
• 
  Must be able to understand medical terminology
