Care Coordinator - Court-Based Treatment & Diversion

Court Based Treatment & Diversion is seeking a Care Coordinator! The Care Coordinator provides services for individuals with mental illness, substance use, or co-occurring disorders to assist them in navigating and access services through screening, coordination, and referral. Works as part of the Court Based Treatment and Diversion team to identify individuals in two local courts to coordinate services for clients, ensure compliance with court approved treatment plan. Also provides discharge services and active follow-up to engage the client in appropriate services.

Starting Hourly Rate: $18.75 Hourly-Depending on Experience

Education/Experience Required:

• High school diploma is required with at least one year of experience in a setting working with individuals with substance use disorders and/or possible co-occurring mental illness.

• Associates or Bachelor’s degree in education, social work, psychology or human services related field is preferred.

• Prior experience working in community behavioral health and demonstrated ability to successfully collaborate with criminal justice partners preferred.
