Position Overview

Full Circle Home Care is seeking a dependable and mature Care Coordinator to support daily operations across our Home Care and DDA programs. This position is ideal for experienced caregivers or DSPs who are ready to transition into an office-based leadership support role while continuing to serve our clients with excellence.

This role works directly alongside our Operations Manager and is essential to ensuring staffing stability, documentation accuracy, and continuity of care.

This is an entry-level operations support position within a growing healthcare organization. We are looking for someone who understands urgency, accountability, and the importance of strong coordination in a fast-paced environment.

Position Responsibilities

• Coordinate caregiver and DSP schedules to ensure service coverage
• Assist with interviewing, screening, and onboarding caregivers
• Communicate professionally with clients, families, and field staff
• Maintain compliance documentation and credential tracking
• Support service authorizations and care plan updates
• Assist the Operations Manager with daily follow-up, problem-solving, and staffing adjustments
• Ensure staffing stability, quality assurance standards, documentation accuracy, and continuity of care

Team Culture & Field Support Expectation

At Full Circle Home Care, no one is above ensuring client care. In the event of urgent call-outs or staffing shortages, team members may be required to assist in the field to maintain service continuity.

Field support is not a daily function of this role; however, flexibility and a team-first mindset are required. Our leadership, including ownership, steps in when necessary — and we expect the same accountability from every team member.

Qualifications

• Experience in home care and/or DDA services strongly preferred
• Working knowledge of caregiver staffing, scheduling, and service coordination preferred
• Professional, dependable, and solutions-oriented
• Strong organizational and communication skills
• Ability to manage multiple tasks in a fast-paced environment
• Valid driver’s license and reliable transportation required
• Must meet eligibility requirements to provide caregiver/DSP coverage if needed
