Care Coordinator - Mental Health Case Management

Position Overview: The Care Coordinator at You First Health Systems will play a critical role in supporting individuals with mental health needs by providing case management services. The primary responsibility is to ensure clients receive the necessary care and resources to improve their mental health and overall well-being. This position involves coordinating care, developing treatment plans, and connecting clients with community resources.

Key Responsibilities:

  • Client Assessment and Intake:
  • Conduct assessments of clients' mental health, medical history, and social needs.
  • Conduct Child and Adolescent Needs (CAN) assessments to identify the specific needs and strengths of younger clients.
  • Develop and maintain individualized care plans based on assessment findings and client goals.
  • Ensure timely and accurate documentation of all client interactions and care plans.
  • Case Management:
  • Coordinate and monitor services for clients, including medical appointments, therapy sessions, and community resources.
  • Advocate for clients' needs within the healthcare and social service systems.
  • Regularly review and update care plans to reflect clients' progress and changing needs.
  • Is employed by the CCO to provide care coordination services to participants.
  • Provides management of the Plan of Care (POC) and facilitation of the Child and Family Team (CFT) meetings.
  • Resource Coordination:
  • Identify and connect clients with appropriate community resources, such as housing, employment, education, and financial assistance.
  • Establish and maintain relationships with service providers and community organizations.
  • Provide clients with information and referrals to relevant support services.
  • Client Support and Education:
  • Offer emotional support and crisis intervention to clients as needed.
  • Educate clients and their families about mental health conditions, treatment options, and self-care strategies.
  • Facilitate support groups and educational workshops for clients and their families.
  • Collaboration and Communication:
  • Collaborate with multidisciplinary teams, including therapists, psychiatrists, social workers, and medical professionals.
  • Participate in case conferences and team meetings to discuss client progress and develop coordinated care plans.
  • Communicate effectively with clients, families, and service providers to ensure continuity of care.
  • Compliance and Documentation:
  • Ensure compliance with all relevant laws, regulations, and organizational policies.
  • Maintain accurate and up-to-date client records in accordance with confidentiality and privacy standards.
  • Prepare reports and documentation as required by funding sources and regulatory agencies.

Qualifications:

  • Bachelor's degree in Social Work, Psychology, Counseling, or a related field (Master's degree preferred), and has met the Department’s training requirements for care coordinators; or
  • High school diploma or equivalency, is 21 years old or older, and was a participant in, or is a direct caregiver, or was a direct caregiver of an individual who received services from the public and child- and family-serving system, and meets the training and certification requirements for care coordinators as set forth by the Department.
  • Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or equivalent credential is preferred.
  • Minimum of 1 year of experience in mental health case management or a related field.
  • Strong understanding of mental health conditions, treatment modalities, and community resources.
  • Experience in conducting Child and Adolescent Needs (CAN) assessments.
  • Excellent interpersonal, communication, and organizational skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency in electronic health records (EHR) and other relevant software.

Working Conditions:

  • Mostly remote with occasional travel to office, client homes, community agencies, and service providers as needed.
  • Flexible working hours, including occasional evenings and weekends, to accommodate clients' needs.

Job Types: Part-time, Contract

Pay: $20.00 - $25.00 per hour
