Interim Care Coordinator Nurse

POSITION SUMMARY

The purpose of your job position as Care Coordinator Nurse is to provide direct nursing care to residents, including the coordination of care that is safe, timely, effective, efficient, equitable, and resident-centered. You are responsible for updating the resident care plans according to the residents condition in coordination with other members of the nursing team. You are also responsible for the education of residents and families on how to follow the current treatment plan. You will use your direct care nursing expertise to serve as the resident/family advocate and act as liaison between medical professionals, payers and all other stakeholders involved in medical care coordination and execution of the resident’s patient care plan, including the Administrator, Director of Nursing and other staff.

ESSENTIAL DUTIES AND RESPONSIBILITIES

1. Execute the patient care plan.

2. Utilizing your direct care nursing expertise, contribute to the development of a safe clinical plan of care, timely, effective, efficient, equitable, and resident-centered.

3. Review medical records of each assigned resident daily in order to accurately manage resident’s progress in accordance with their patient care plan.

4. Coordinate treatment plans and case management with IDT members.

5. Help residents achieve clinical wellness and autonomy.

6. Help residents and family members make informed decisions with the clinical care team by acting as their advocate regarding their clinical status and treatment options.

7. Build trusting relationships with patients to keep track of their progress.

8. Assess and address motivational and psychosocial issues with residents and serve as resident advocate in collaborate with Social Services as necessary.

9. Utilizing your direct care nursing expertise, record cases information, complete accurately all necessary forms and produce statistical reports.

10. As a nurse and utilizing your direct care nursing expertise, participate in the coordination of care with all payers.

11. Follow up with residents’ post-discharge.

12. Communicate by phone or in writing regarding the resident’s status and changes in care plan to all relevant parties.

13. Using your direct care nursing expertise, document resident progress and document all payer interactions regarding resident progress and expected outcomes.

14. Report any occupational exposures to blood, body fluids, or other hazardous materials to a supervisor immediately.

15. Protect each resident's file and data with confidentially per HIPAA standards. 16. Abide with all facility policies and procedures including not disclosing user ID codes and passwords.

The Care Coordinator Nurse serves as the resident advocate and plays the vital role of facilitating care coordination between medical professionals and all other stakeholders involved in executing the resident’s patient care plan. The Care Coordinator Nurse will treat each resident fairly, kindly, and show them dignity and respect. Such behaviors include:

  • Knocking before entering each resident’s room
  • Referring to each resident by their name
  • Communicating clearly, empathetically, and effectively when speaking to residents, family members, visitors, all staff and governmental agencies
  • Working cooperatively with all departments and multidisciplinary teams
  • Maintaining a positive and calm disposition
  • Demonstrating patience, initiative and willingness to assist residents that may be difficult
  • Relaying all pertinent information concerning a resident’s condition to a charge nurse when required
  • Having a commitment to always doing the right thing

REQUIREMENTS

Education / Licensure Completion of nursing training from accredited school or college Valid nurses license in good standing Valid CPR and BLS card

Qualifications / Experience

Minimum of 1 year case management experience in skilled nursing facility preferred

Must have 5 or more years as a licensed nurse Excellent verbal and written skills Knowledge of Point Click Care Helpful

Working Conditions

May encounter frequent interruptions May be involved with residents, family and government agencies

May be requested to work beyond scheduled working hours at times Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.)

May be necessary to assist in the evacuation of residents during emergency situations

May be exposed to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses Physical Requirements

Must be able to move intermittently throughout the day Working throughout the nursing areas Repetitive hand motion
