A morning might start with a parent calling because they're overwhelmed and don't know what step comes next. By the afternoon, you might be helping connect that same family with the right support, answering a DSP's question in the field, and making sure services are running smoothly for the individuals we serve.

That's the life of a Care Coordinator at Endeavor HCBS.

This role is part advocate, part organizer, and part problem-solver. You help keep the moving pieces of care aligned so that families feel supported and individuals with developmental disabilities receive the services that help them thrive at home and in their communities.

Some days you're coordinating schedules and documentation. Other days you're guiding families through the HCBS system or checking in with team members to make sure everything is on track. Every day, your work quietly makes life easier for someone who needs it.

What the Role Looks Like

• Support families and individuals receiving services

• Coordinate communication between DSPs, families, and the Endeavor team

• Keep documentation organized and compliant with Arizona DDD guidelines

• Help ensure services are delivered the way the planning team intended

Schedule
Monday–Friday | 8:00 AM – 4:00 PM

We're Looking For Someone Who

• Communicates clearly and compassionately

• Stays organized when managing multiple moving parts

• Enjoys helping people solve problems

• Wants to build a career supporting the I/DD community

Experience in HCBS, social services, healthcare, or education is helpful-but not required. We provide training for people who are motivated to learn and care about the work.

Requirements

• Level One Fingerprint Clearance Card (or ability to obtain)

• Valid driver's license and reliable transportation

• Ability to pass CPS/APS registry checks

• Comfortable using Apple devices and Google Workspace
