The Opportunity

We are looking for experienced LPN’s who have current compact multistate licensure and will support our remote chronic care management (CCM) and remote patient monitoring (RPM) operations in the US. The LPN will work with a team of other nurses and healthcare assistants who work remotely in their care of a designated roster of patients. The role assesses patient conditions, and then plans, implements, provides, and documents patient care in a manner that follows professional standards and outlined practices.

Responsibilities

• Manage ongoing relationships and communication with assigned patients.
• Follow up on patient needs, coordinate care plans, and monitor medication compliance.
• Document interactions, notes, and progress accurately in the ColigoMed portal.
• Escalate clinical concerns promptly to the assigned healthcare provider.
• Build effective relationships with the remote care team and medical providers and provide support as needed
• Work with health providers to provide feedback on patients as needed and any required follow-up
• Develop and maintain a deep knowledge of the remote care model and ColigoMed product and new developments in remote RPM and CCM solutions
• Conduct training, coaching and debriefing of team members as needed in a productive and constructive manner
• Perform other duties and responsibilities as assigned

Education/Licenses/Experience

• LPN/LVN with an active compact multistate license is mandatory
• Minimum of 6-10 years of nursing experience including chronic disease management
• 1 + years of relevant experience specifically in remote chronic care management, remote patient monitoring or telehealth using a healthcare digital platform
• Proven experience and knowledge with a range of different EMR systems
• Demonstrated stable employment history

Knowledge and Skills

• Ability to work remotely & effectively in a remote team to deliver required results
• Excellent organizational skills including prioritizing, scheduling, time management, and meeting deadlines
• Strong comprehension and communication skills to receive and convey information effectively to patients, team members and medical providers.
• Proven ability to build relationships and earn trust with patients and healthcare providers and to manage changing priorities in an effective and organized manner
• Broad clinical knowledge to help support patient questions, comments and build confidence with patients and medical providers
• Maintain patients’ privacy, confidentiality and adhere to ethical and legal requirements
• A team player with high level collaboration skills
• Bilingual skills in Spanish are preferred

Note: A background check will be required and after successful training, this position is a remote (from home) role. You must have a conducive place at home to perform professional, quiet, and confidential work.

Working hours

Full time, 40 hours per week (Monday to Friday) working an 8-hour shift from 8.00am to 5.00pm ET is a mandatory requirement.

Note: As requirements and patient location evolves over time, flexibility in day shift working hours may be required.

Compensation:

This is a 1099 role at an hourly rate ranging from $22.00 to $25.00

The commencing position in the hourly rate range will take into account an assessment based on factors including the candidate's level of experience, education, skills, and location.

Note: This is a 1099 role paid on an hourly rate with no benefits. Over an initial six (6) months period ColigoMed will assess and consider transition to a full-time W2 employment role.
