Care Management Supervisor – Licensed (Supervisory Lead)
Position Overview
The Admissions Behavioral Health Clinician (Licensed/Supervisory) is a senior clinical role within Paragon's Admissions Department, reporting directly to the Admissions Director. This position serves as the clinical hub of the admissions process — leading the systematic review of incoming screening and assessment materials, categorizing client acuity across the lifespan, and orchestrating the care coordination workflow that bridges intake with ongoing clinical services.
This clinician brings deep expertise in screening, clinical conceptualization, and level-of-care determination for medium- and high-acuity clients, and leads two care management/care coordination teams to ensure each client is thoroughly prepared for transition into their assigned clinical track. The ideal candidate is highly detail-oriented, adaptable, and skilled at translating complex assessment data into actionable care priorities — all while maintaining a calm, decisive presence in a fast-paced admissions environment.
Role Responsibilities
Clinical Screening & Admissions Review
- Review and clinically interpret all incoming screening and admissions materials, including PRAPARE social determinants data (food security, housing, financial stability, transportation, social supports, and more).
- Conduct and/or oversee comprehensive behavioral health assessments — including evaluation of psychiatric acuity, suicidality, homicidality, substance use, trauma history, and psychosocial risk factors — across the full lifespan (child, adolescent, adult, older adult).
- Apply evidence-informed level-of-care criteria (LOCUS, CALOCUS, ASAM) to determine appropriate service placement.
- Categorize client acuity and systematically identify and prioritize care needs, including:
- Medication evaluation and referral to the medical team
- Benefits acquisition and insurance eligibility coordination
- Housing needs identification and stabilization support
- Financial resource navigation
- Medication reconciliation and coordination with prescribers
- Crisis mitigation planning and safety coordination
- Translate assessment findings into clear care priorities and task assignments for care management staff.
- Support and contribute to individualized treatment planning for high-acuity clients transitioning into ongoing clinical tracks.
Care Coordination Oversight
- Oversee the workflow of two care management/care coordination teams, ensuring tasks are appropriately assigned, monitored, and completed in alignment with each client's identified needs and acuity level.
- Supervise care managers in implementing PRAPARE-informed interventions, social determinants screening follow-through, and coordination with community resources.
- Ensure clients are fully prepared — clinically, logistically, and socially — for a smooth handoff into their assigned ongoing care track (e.g., IOP, ACT, in-home services, outpatient).
- Maintain close communication with receiving clinical teams to support seamless, well-documented transitions.
- Identify systemic barriers in the admissions-to-care pathway and collaborate with the Admissions Director to develop solutions.
Supervision & Team Leadership
- Provide direct clinical supervision and professional mentorship to care management staff across two teams.
- Lead regular team meetings, case consultations, and workflow check-ins to ensure quality, consistency, and staff development.
- Model clinical best practices in case conceptualization, documentation, and client engagement.
- Support staff in navigating complex, high-acuity situations with evidence-based guidance and clinical grounding.
- Participate in admissions leadership meetings and contribute to program development and quality improvement initiatives.
Documentation, Compliance & Quality
- Ensure accurate, timely, and clinically thorough documentation in accordance with agency, state, and payer standards.
- Maintain compliance with HIPAA and all applicable confidentiality regulations.
- Participate in quality assurance processes, including audits of admissions documentation and care coordination records.
- Engage in ongoing professional development and agency-required training (e.g., Crisis Prevention and Intervention).
