Care Manager / Medical Assistant

We are looking for an energetic, compassionate, flexible, and responsible person to manage our high risk and chronically ill medical patients. Responsibilities include coordinating patient care to improve quality of care through the use of internal and external resources, resulting in enhanced quality and cost effective outcomes. This self motivated individual will act as an advocate for our patients healthcare needs, and coordinate care in order to connect the patient with other health care partners.

Job functions include but not limited to:

- Collaborate with providers and staff in identifying appropriate patients for Care Management.

- Formulate and implement a Care Management Plan of Care, in conjunction with the provider, that addresses the patients identified needs by assessing the patient / family needs, issues, resources and care goals; determining the choices available to the patient; educating the patient / family on the choices available.

- Periodically evaluate the effectiveness of the plan in meeting established goals; revise the plan as needed to reflect changing needs, issues and goals.

- Collaborate with the health care team to revise the care management plan when changes occur. Initiate care conferences to discuss multidisciplinary team responsibilities, patient progress, new problems, etc.

- Identify and effectively utilize community resources to meet the needs of patients / families. Facilitate patient access to community resources as appropriate.

- Promotes patient and family self management.

- Coordinate with external providers on medication, plans of care, etc.

- Review TOC/ED lists and contact patient within 2 days of discharge. Encourage patients to be seen in office.

- Keep primary provider informed of any changes in admissions, discharges, acute illnesses, medication issues, etc.

Job Type: Full-time

Pay: $16.00 - $18.00 per hour

Experience:

  • EMR systems: 1 year (Preferred)
  • Vital signs: 1 year (Preferred)

License/Certification:

  • BLS Certification (Preferred)
  • Certified Medical Assistant (Preferred)
