Care Transition Coordinator (Hospice Sales)

Overview
We are seeking a dynamic and dedicated Care Transitions Coordinator to join our hospice team. In this vital role, you will facilitate seamless patient transitions from hospital to home or other care settings, ensuring continuity of care and optimal patient outcomes. Your expertise will help bridge communication gaps between hospitals, outpatient providers, and community resources, promoting safe and efficient admission to hospice. This position offers an exciting opportunity to make a meaningful impact on patient sacred journeys through proactive coordination and comprehensive management.

Duties

  • Collaborate with multidisciplinary teams including nursing, social work, physicians, and case managers to develop individualized discharge plans.
  • Conduct thorough patient assessments to identify needs related to hospice care.
  • Manage intake processes and utilize electronic health record (EHR) systems to document patient information accurately while adhering to HIPAA regulations.
  • Facilitate communication between patients, families, and community providers to ensure understanding of discharge/admission instructions and follow-up care.

Requirements

  • Strong background in medical sales and/or nursing, social work, or case management with familiarity in hospice care.
  • Ability to navigate complex healthcare environments including acute care facilities and managed care organizations.
  • Excellent communication skills for effective intake interviews and patient service interactions.
