    JOB SUMMARY: The Career Plus Case Manager is responsible for providing support to participants in our Career Plus Program. This includes but is not limited to: educational needs; career elements, including resume writing, job search, and job retention. This position also requires working with DES and keeping all files in compliance with all requirements and timeframes.

    MISSION AND SERVICE:

    ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES: The primary duties and responsibilities include, but are not limited to, the following:

    • Schedule and complete all intakes for potential participants.
    • Process all intake paperwork.
    • Participate in various job fairs and other community events as needed.
    • Keep accurate monthly records for all students and their files, including but not limited to:
    • Employment Assessment, Employment Career Development Plan (ECDP- Initial & Revised), Monthly Case Notes, and Time Logs (verifying total hours reported in the billing month), Receipts for supportive services issued, and Demographics Statistics.
    • Staff all support service requests with the Program Director to ensure the participant is Referred and Verified for SNAP CAN.
    • Execute Support service requests after approved and ensure all necessary documentation is completed.
    • Assist the Program Director with DES student file audits.
    • Complete SNAP monthly paperwork due by the 1st Monday of every month.
    • Ensure all participant files are kept up to date.
    • Work with participants to identify any life and/or employment hurdles, and complete the service plan monthly.
    • Meet with all students at least monthly to update their service plan.
    • Assist students with resume writing and the job search process.
    • Keep regular contact with students for on-going support.
    • Be able to help and support students with emergencies after hours as needed.

    Miscellaneous Duties

      • Provide input on annual reviews when requested.
      • Draft letters and develop forms to help communicate and create a positive image with the public and peers.
      • Personally grow in leadership and spiritual life to exemplify the characteristics of a cultural champion.
      • Represent Miracle Center and GAP Ministries to the public in a positive, professional, and responsible manner..
      • Follow the agency chain of command in a responsible and supportive manner
      • Be able to accept constructive feedback and suggestions for improvement.
      • Comply with Miracle Center and state confidentiality policy.
      • Perform other appropriate tasks as assigned by the Program or Executive Director or CEO.

      QUALIFICATIONS:

    • 2 Years full-time experience performing supervisory responsibilities or highly responsible work relating to tasks assigned to this position.
    • Proven ability to effectively function in a multi-faceted agency in a teamwork manner. Be able to respect and work cooperatively with a variety of persons, including coworkers, members of the board, representatives of social service agencies, the public, and volunteers.
    • Understand the principles and flow of the service plan process and be able to execute.
    • Ability to intervene in crisis situations and/or attempt to avert potential crises.
    • Requires strong oral and written communication skills.
    • Ability to work well with ever changing priorities and/or situations.
    • Proficient in the use of a personal computer and corresponding programs; preferably experience with Microsoft Office Suite, Gmail and Google Calendar or similar web-based and computer programs.
    • Have personal attitude, conduct and standards representing those qualities and values reflected in the ministry that clients can respect and emulate.
    • Must be a self-starter who can work well with people at all levels both in and out of the office. Can stay calm in tense situations and utilize common sense and logic.

    REQUIREMENTS:

    • Hold a Bachelor’s degree in Social Work, Psychology, Social Work or a related field; or an Associates degree, plus at least 2 years of experience in case management and providing services to participants; or a High school diploma plus 4 years of experience in case management.
    • Must be at least 22 years of age.
    • Successfully pass FBI Criminal Background Check and procure a Level 1 Fingerprint Clearance Card.
    • Successful clearance of Arizona Department of Child Services Central Registry database, negative drug screen,and verification of legal ability to work in the United States.
    • Two supportive personal reference checks of those listed on application as well as employment verification of the last 2 years of most recent employment.
    • Must have a valid driver license in effect for at least 5 years, meet the insurability requirements of Miracle Center and provide a 5-year safe driving record (MVR) from the Department of Motor Vehicles.
    • Complete onboarding documents and process.

    o Miracle Center is a drug-free workplace and requires drug screening of all new employees as well as random screenings to ensure enforcement of this policy.
