Caregiver

Currently hiring a full time noc (overnight) shift caregiver at our Monmouth, OR location. hours are typically 8pm - 8am or 12am-8am, but varies depending on needs.

We are also hiring a part time/on call caregiver for some day shift and swing shift.

Overview
At Acumen Foster Homes we specialize in caring for people with Dementia and Challenging Behaviors. We have two locations, one in Sheridan, OR and one in Monmouth, OR. At both homes we focus on providing person centered care with frequent 1:1 support.

Responsibilities

• Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary
• Monitor patients’ health by taking vital signs, observing changes in condition, and reporting findings accurately to healthcare professionals
• Support medication administration in accordance with care plans and HIPAA regulations to ensure safety and compliance
• Prepare nutritious meals tailored to individual dietary needs and assist with meal planning and cooking
• Maintain a clean and safe environment through regular cleaning duties, including laundry, disinfecting surfaces, and organizing personal spaces
• Document care activities meticulously, including patient observations, care plan updates, and incident reports
• Provide emotional support and companionship to foster a positive environment for residents or clients with developmental disabilities, dementia, or other conditions such as Alzheimer’s or autism

Qualifications

• Prior experience in home health, assisted living, nursing homes, or group homes is highly desirable
• Knowledge of patient observation techniques and ability to monitor vital signs accurately
• Certification in CPR and First Aid; additional training in Alzheimer's care, dementia care, hospice care or memory care is preferred
• Familiarity with HIPAA regulations to ensure confidentiality of patient information
• Ability to perform heavy lifting safely and operate assistive devices like Hoyer lifts or resident lifts when needed
• Strong communication skills for working effectively with clients, families, and healthcare teams
• Experience working with individuals with disabilities or developmental challenges is a plus
