Caregiver/ CNA/ HHA

Responsibilities

  • Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other equipment
  • Monitor and record vital signs accurately while maintaining strict adherence to HIPAA regulations to ensure patient confidentiality
  • Observe patients closely for changes in condition, including signs of dementia, Alzheimer's disease, or other memory impairments, and report findings promptly
  • Administer medications safely following prescribed protocols and document all medical records using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
  • Assist with specialized care needs such as catheter care, hospice support, dementia care, and Alzheimer's care to enhance quality of life
  • Support patients with nutritional needs by preparing meals tailored to dietary restrictions and assisting with feeding when necessary
  • Maintain a clean and safe environment by performing light housekeeping tasks such as laundry, cleaning, and ensuring infection control standards are met

Experience

  • Prior experience working in long-term care facilities like nursing homes or assisted living communities is highly valued
  • Demonstrated ability to work effectively with individuals with developmental disabilities or disabilities requiring specialized care
  • Familiarity with medical terminology, medical documentation practices, and HIPAA compliance is essential
  • Hands-on experience with home health services including home & community care is preferred
  • Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care training or dementia-specific certifications are a plus
  • Proven ability to operate EMR/EHR systems efficiently for accurate record keeping and communication
  • Strong caregiving skills complemented by patience, empathy, and excellent communication abilities
