Southwest Louisiana Independence Center (SLIC) is seeking a dependable and detail-oriented Case Coordinator / Case Manager to join our Lake Charles team. This role focuses on coordinating and monitoring home and community-based services for individuals with disabilities and older adults throughout the Lake Charles area, ensuring quality care and support. Ideal candidates have experience in case management, social services, or healthcare coordination and are comfortable working independently while maintaining strong organization and documentation standards.

Key Responsibilities: Case Coordinator / Case Manager – Full Time – Lake Charles, LA

• Monitor home and community-based services through monthly phone contact and quarterly home visits

• Maintain timely, accurate documentation and ensure all required records are complete and compliant

• Identify changes in individual needs and coordinate appropriate service adjustments

• Address and resolve concerns related to service delivery, staffing, or scheduling

• Recruit, hire, and train Direct Service Workers (DSWs)

• Monitor DSW performance to ensure services are delivered according to the plan of care

• Conduct unannounced visits to evaluate service quality and compliance

• Communicate professionally with individuals, families, staff, and community partners

• Participate in an after-hours on-call rotation as scheduled

Qualifications & Requirements: Case Coordinator / Case Manager – Full Time – Lake Charles, LA

• Minimum of 2 years experience in case management, social services, healthcare, disability services, or related field

• Demonstrated ability to work independently and manage competing priorities

• Strong organizational skills and attention to detail

• Professional written and verbal communication skills

• Ability to maintain confidentiality and professional boundaries

• Knowledge of community resources preferred

• Experience with waiver programs preferred but not required:

  • New Opportunities Waiver (NOW)

  • Children’s Choice Waiver (CC)

  • Community Choice Waiver (CCW)

  • Supports Waiver

  • EPSDT

• Starting Pay: $15.50 per hour (based on experience)
