Case Management and Care Placement Specialist

Are you passionate about helping seniors and our homeless population transition safely and successfully from skilled nursing facilities into the community or assisted living? We are seeking a highly motivated, professional, and adaptable individual to join our growing team.

This is not an entry-level role. We are looking for someone who understands the healthcare industry, can communicate confidently with families and healthcare professionals, and thrives in a fast-paced environment where every day is different.

What We’re Looking For

The ideal candidate has experience working with:

• Elderly/Homeless populations
• Case management
• Skilled nursing discharge planning
• Hospital discharge planners
• Assisted living and community placement
• Healthcare coordination and family support

You must be:

• Professional, dependable, and responsible
• Able to pivot between multiple roles and responsibilities
• Comfortable communicating with hospitals, skilled nursing facilities, families, and care providers
• Skilled in healthcare terminology and documentation
• Strong in relationship-building and marketing/outreach
• Flexible with hours, including weekends when needed
• Self-motivated and able to work independently

Requirements

• Valid driver’s license
• Ability and willingness to commute throughout:
• Bay Area
• Central Valley
• Sacramento region
• Reliable, organized, and detail-oriented
• Strong interpersonal and communication skills
• Experience in healthcare placement, senior care, discharge planning, or related field preferred
• Bilingual skills are highly valued and considered a strong plus.
