Case Management Coordinator

Job Summary
We are seeking a dynamic and detail-oriented Case Management Coordinator to join our SUD and Behavioral health team. In this vital role, you will coordinate comprehensive patient care plans, facilitate seamless discharge processes, and ensure optimal utilization of resources across various mental health settings. Your expertise will help improve patient outcomes, enhance communication among multidisciplinary teams, and support patients and their families through their SUD treatment, and Behavioral Health journeys.

Duties

  • Collaborate with SUD and Mental Health providers, social workers, and family members to develop individualized care plans tailored to each patient’s needs.
  • Conduct thorough patient intake assessments, including reviewing mental health histories and current health status.
  • Utilize electronic health record (EHR) systems to document patient interactions, treatment plans, and progress accurately while maintaining compliance with HIPAA regulations.
  • Assist in managing cases involving behavioral health by providing targeted support and oversight.

Skills

  • Experience working in mental health, sud treatment clinical settings
  • Knowledge of managed care processes, discharge planning strategies, and social work principles related to behavioral health and sud treatment services.
  • Familiarity with EMR/EHR systems used for documentation, case tracking, and communication within behaviorral health clinical settings.
  • Excellent organizational skills with the ability to prioritize multiple complex cases efficiently.
  • Strong interpersonal skills for effective collaboration with mental health professionals, patients, families, and community agencies.

Pay: $24.40 - $29.38 per hour

Work Location: Hybrid remote in Grosse Pointe, MI 48230
