Muncie Primary Care is a family-owned, patient-centered practice that has proudly served the community since 1981. We are seeking two compassionate and detail-oriented Registered Nurses (RNs) to join our growing Chronic Care Management (CCM) team. These are onsite weekday-only positions—no weekends, no holidays—offering a chance to focus on meaningful, relationship-based care for patients with chronic conditions.

About the Role

This RN will play a key role in our Chronic Care IQ platform and electronic health record (EHR) system to support patients with multiple chronic conditions. You’ll work directly with patients between visits, helping coordinate care and support long-term health outcomes.


Chronic Care Management responsibilities include:

  • Managing a patient panel of up to 350 patients per month
  • Accurately logging time spent on care coordination using Chronic Care IQ and our EHR
  • Conducting ongoing assessments of medical, functional, and psychosocial needs
  • Supporting timely access to recommended preventive services
  • Medication reconciliation, adherence checks, and interaction reviews
  • Educating and empowering patients in self-management of their conditions
  • Coordinating care with internal providers and community-based services
  • Maintaining thorough documentation of all care management interactions


In addition , you may support other RN tasks as needed:

  • Telephone triage
  • Patient questions and education with provider support
  • Reviewing and communicating test results and referrals
  • Medication prior authorizations and prescription refills
  • Population health and care coordination activities


What We’re Looking For

We’re looking for a highly organized, tech-savvy RN who thrives in a structured, patient-centered environment. The ideal candidate:

  • Is confident using healthcare technology to manage large patient panels and track care
  • Demonstrates exceptional attention to detail and documentation
  • Is results-driven, proactive, and efficient in managing time and care coordination
  • Communicates clearly and empathetically with patients and providers
  • Excels at working independently while contributing to a collaborative team


Required :

  • Associate’s or Bachelor’s degree in Nursing (ASN or BSN)
  • Current Indiana RN license
  • Graduate of a nationally accredited nursing program

Preferred :

  • 2+ years of experience working with adult patients
  • Experience with chronic care, case management, or population health
  • Familiarity with EHR systems and/or Chronic Care IQ
