Description:
Overtown Youth Center is seeking a dedicated and experienced Case Management Program Manager to join our dynamic team. This role is ideal for a compassionate and strategic professional who is passionate about supporting families in under-resourced communities and committed to delivering high-quality, trauma-informed, and culturally responsive services. As the Case Management Program Manager, you will provide day-to-day leadership and oversight of the case management team, ensuring that participants receive coordinated support that promotes stability, well-being, and long-term success.
Through effective supervision, program oversight, and collaborative problem-solving, you will guide staff in delivering impactful services while maintaining strong program quality and accountability. In this role, you will also foster a supportive and mission-driven team environment that prioritizes family-centered care, strengthens community connections, and advances the organization’s goals and outcomes.
What you would do:
- Lead and supervise the case management team, providing coaching, performance feedback, and professional development to ensure delivery of high-quality service.
- Oversee case management services and family success planning, ensuring support is trauma-informed, culturally responsive, and aligned with program goals.
- Support staff in navigating complex cases and crises, helping remove barriers impacting youth and families.
- Monitor program quality and outcomes, including reviewing documentation, tracking service delivery metrics, and ensuring compliance with funder and agency requirements.
- Strengthen partnerships with community providers, schools, and referral agencies to coordinate wraparound services for families.
- Support program operations and continuous improvement, including staff onboarding, workflow coordination, and participation in quality improvement efforts.
- Ensure accurate data collection and reporting, maintaining documentation standards and alignment with grant and contract requirements.
Requirements:
Who we’re looking for:
- Bachelor’s degree in Social Work, Human Services, Psychology, Education, or a related field required; Master’s degree preferred.
- 3-5 years of experience in case management or human services, including experience supervising or leading staff.
- Experience supporting youth and families in under-resourced or high-need communities.
- Strong knowledge of trauma-informed care, strengths-based practice, and wraparound service models.
- Excellent communication, documentation, and organizational skills, with the ability to manage multiple cases and priorities.
- Experience working in nonprofit or community-based organizations and collaborating with community partners.
- Familiarity with case management databases and outcome tracking systems to support reporting and program evaluations.
- Bilingual (English/Spanish or another relevant language) preferred.
- Valid driver’s license, reliable transportation, and the ability to pass drug screening and Level 2 background check.
- Ability to work flexible hours, including weekends, as needed.
