Definition of Position: This is a full-time, 40 hour per week position that manages the daily operations of a case management unit providing services to individuals with mental illness living in scattered site independent housing programs. This person is responsible for directing a designated group of team members in their day-to-day operations. The person in this role will report directly to the Case Management Services Director.
General Responsibilities Include:
- Supervision of case management and support staffs’ daily activities and schedules.
- Coordination of service delivery to meet client needs, program goals and contract objectives.
- Ensuring service outcomes and specific service delivery metrics are met.
- Attend Agency, County and community meetings as assigned or required.
- Ensure programs are staffed appropriately.
Specific Responsibilities Include:
- Recruit and train new case management and support staff.
- Provide direction to case managers in service delivery and coordination to meet client needs, program goals, and contract objectives.
- Ensure case managers complete and maintain files and documentation.
- Monitor client-related paperwork to ensure forms are complete and clients are registered in the system.
- In collaboration with case manager and clinical supervisor re-evaluate client needs on a regular basis.
- Review staff caseloads and transfers clients among case managers to ensure highest level of coverage and service possible.
- Ensure case manager know and effectively use agency and community resources.
- Provide back-up support and services to clients when case managers are unavailable.
- Ensure individual Case Manager productivity reaches or exceeds minimum Agency standards.
- Provide support to Case Managers and support staff on a regular basis.
- Meet regularly with Case Managers to review their progress, areas of concern, opportunities for growth and educational activities that improve skills and abilities.
- Complete reports for funders and others as required.
Qualifications
- B.A. in psychology, social work, or related field.
- Experience in program management
- Experience working in a multi-disciplinary team
- Experience working with people with psychiatric disabilities
- Experience developing client assessments and treatment plans
- Experience with MediCal documentation
- Ability to be flexible
- Ability to work with a diverse population
- Good computer and documentation skills
- Safe driving record and proof of insurance
- Must have a car, transporting clients is a requirement
Knowledge and Skills:
- Ability to write and speak clearly, easily communicating complex ideas across multiple platforms.
- Ability to remain poised in stressful situations and communicate diplomatically via telephone, computer, and written correspondence.
- Ability to skillfully manage multiple, complex projects and competing priorities concurrently while working under pressure to meet deadlines.

General Agency Expectations:

- Comply with all program and agency policies & guidelines when serving clients.
- Develop and maintain good working relationships with all MHA employees.
- Become familiar with other agencies and service providers in the county and develop good working relationships with these resources to provide clients with the advocacy and support they need.
- Maintain regular contact with supervisor. Communicate daily schedule and necessary information in a timely manner.
- Actively plan and participate in all-staff meetings, trainings, retreats, and other activities as requested. Participate in ongoing quality improvement activities.
- Assume responsibility for other tasks as needed.
Pay: $32.00 - $36.00 per hour
