Case Management Social Worker

We are seeking a compassionate and detail-oriented Case Manager to coordinate patient care and support safe, effective transitions across the continuum of services. In this healthcare Case Manager role, you will assess patient needs, collaborate with interdisciplinary teams, and ensure access to appropriate resources. Your work will directly support patient well-being, recovery, and continuity of care.

What You’ll Do

Care Coordination & Patient Support

• Coordinate care and services to maximize each patient’s recuperative potential

• Conduct intake assessments with patients and families to identify needs

• Develop individualized treatment plans with the interdisciplinary team

• Arrange follow-up services, transportation, and community resource connections

Patient & Family Experience

• Ensure families feel supported and informed throughout the care process

• Counsel patients and families on interpersonal, financial, and rehabilitation concerns

• Provide information on Advance Directives and assist with completion as needed

Cross-Functional Collaboration

• Partner with healthcare teams to review progress and determine next steps

• Coordinate admissions and discharges for the Transitional Care Program

• Collaborate with internal and external partners to ensure smooth transitions of care

Systems & Administration

• Verify insurance coverage and assist patients with financial or resource barriers

• Maintain accurate documentation in patient databases and hospital systems

• Ensure compliance with HIPAA, Joint Commission standards, and regulatory requirements

Work Environment

This is a 100% onsite position at Magruder Hospital in Port Clinton, OH.

Required

• Bachelor’s degree in a Human Services field with licensure-eligible credentials

• Supervised field experience in case management, social work, health, or behavioral health

• Strong communication and counseling skills

• Proficiency with Microsoft Office and patient database systems

Preferred

• Experience in a healthcare or hospital setting

• Commission for Case Management Certification or Certified Case Manager

• Familiarity with local community resources and geriatric care needs
