We are seeking a Case Management Supervisor to support and guide a dedicated team serving individuals in Los Angeles County’s Medi-Cal DMH residential mental health programs.
This is a meaningful leadership opportunity to work directly with individuals transitioning from higher levels of care—including hospital settings—and help them stabilize, access critical resources, and successfully reintegrate into the community.

What You’ll Do
- Provide hands-on leadership and support to case management staff across two residential programs
- Guide and strengthen discharge planning efforts to ensure safe and sustainable client transitions
- Support team members in connecting clients to:
- Housing resources
- Public benefits
- Medical and mental health services
- Community-based support systems
- Build and maintain strong knowledge of Los Angeles County programs, including FSP, ERS, IMD placements, and other key resources
- Ensure timely, accurate, and compliant documentation within EMR systems
- Act as a resource and problem-solver for complex client situations
What You Bring
- Prior experience in case management (required)
- Strong background in discharge planning and care coordination (required)
- Knowledge of Los Angeles County mental health and housing systems (strongly preferred)
- Experience working with Medi-Cal / DMH populations (preferred)
- Behavioral health or residential treatment experience (a plus)
- Ability to support a team while also staying engaged in day-to-day operations

Additional Requirements
- Must be based in Los Angeles County
- Must be able to travel between Sylmar and Downey locations
- Must pass or have previously passed a Live Scan background check
- This is afully in-person role

Pay: $25.00 - $30.00 per hour
Work Location: In person
