The Case Manager/Service Coordinator plays a vital role in assisting individuals in accessing and coordinating services that support independence, community integration, health, and overall well-being. This position is responsible for coordinating, advocating for, and monitoring services identified within the participant’s Service Plan to ensure all needs are appropriately addressed.
Services may include Medicaid State Plan services, HCBS Waiver services, medical and behavioral health supports, educational and vocational services, housing resources, and other community-based supports. The Service Coordinator works collaboratively with participants, families, providers, and interdisciplinary teams to promote person-centered care and successful outcomes.
The ideal candidate is compassionate, organized, self-motivated, and passionate about supporting individuals in achieving their goals while maintaining their health and safety within the community.
